Dear Parents,

 

I can't believe it's that time of year again! The school year is off to a great start. I hope you had a fun and fantastic summer!

 

I would like to make you aware of an issue that came to our attention during the last school year. When your child returns to school, you may be given a form to fill out requesting your child's insurance or Medicaid information and your consent for a DENTAL SCREENING.

 

PLEASE DO NOT FILL THIS OUT IF YOU WANT US TO CONTINUE TO SEE YOUR CHILD. THIS IS NOT A FREE SCREENING. YOUR CHILD'S BENEFITS WILL BE USED FOR THIS DENTAL SCREENING.

 

ONCE THE BENEFITS ARE USED, THIS LIMITS WHAT CAN BE DONE IN ANY DENTAL OFFICE WITHOUT YOU HAVING TO PAY FOR SERVICES.

 

If you have already signed this form you will need to contact the school and let them know you do not want your child seen at the school for a dental screening.
